Call Customer Service (toll free) at 866-DalySvc, have your serial number ready, and they should be able to send one along with the swivel pack.

Only give us a day or two before you call. Our phone system tanked today and although we are receiving calls, the whole thing is jury rigged and can barely keep up. All the lines may crash any moment now as we scramble to get new equipment installed. :mad:
